Statement of Employment Insurance Benefits T4E - Spouse’s Income
To ensure your maximum combined refund, we should prepare your spouse's tax return as well, or you can provide a copy of your spouse's completed tax return. - Spouse or Child support payments received
Report all support payments that you received, both taxable and non-taxable. - Universal Child Care Benefit (UCCB)
Married or common-law, must be claimed by the spouse with the lower net income. (RC62 slip) - Pension Income
T4A-slips: T4A (Pension), T4AP (Canada Pension), and T4AOAS (Old Age Security) - Worker Compensation or Social Assistance
Statement of Benefits T5007. Note: if married or common-law, must be claimed by the higher income spouse. - Investment Income
T-slips: T4RSP (retirement fund) T4RIF (income fund), T3, or T5

Payment receipts or insurance statement. Receipts must total more than 3% of net income. - Child Care Expenses

Official tax receipts required if not on T4. Contribution limit from last year's Notice of Assessment. - Union or Professional Dues
Official tax receipts required if not on T4. - Political or Charitable Donations
Submit your "Official receipts for income tax purposes" - Public Transit Passes
Monthly or weekly passes. - Trades People & Apprentice Mechanic Tools Deduction
If you purchased more than $1,000 in tools, you may claim up to $500 if you provide the original receipts.
